GMF Nurses

Our nurse clinicians work closely with your family doctor, in the care of their patients.

Upon referral by one of our doctors, patients registered in our GMF can receive services concerning a variety of health problems including diabetes, high blood pressure, cholesterol problems, medication, chronic illness monitoring, as well as offering PAP tests.

Nurses also offer diagnostic testing for cognitive disorder screening, memory difficulties, counseling and teaching for sexually transmitted infections, contraception, and child development (0-5 years old).

Your doctor has requested a specific test for the nurse to do, you will be contacted by them for an appointment.

Please note that our nurses do not:

Book family practice appointments.
Renew prescriptions.
Copies of results/Reports
Status on Appointments with Specialists/Clinics
Status on Medical forms
